The MOST is a document designed to help healthcare providers honor the treatment wishes of their patients. It is part of a program known as the Physician Orders for Life Sustaining Treatment "POLST" which began in Oregon in 1995. Since its inception the POLST has proven successful in promoting clarity in patients' wishes for end of life care. As a result of this success the POLST paradigm has spread throughout the United States and several other countries with slight variations (POST, MOLST, MOST). In 2011 a coalition of multidisciplinary professionals formed to bring the POLST paradigm to New Mexico. The resulting MOST document allows New Mexico residents and health care provider the ability to have a meaningful conversation regarding healthcare options and to have that conversation converted into a medical order.
