NM MOST and Patients & Families

Advance care planning empowers you to have the type of treatment you wish to receive.  

When you share your wishes in a written document that is available to your healthcare providers, it reduces the emotional stress of last-minute decision making. The end result is that you receive the type of care that is consistent with your values and your loved ones are spared the stress of not knowing what you want. This process is especially critical in the event you are seriously ill, are very frail, or are facing limited life expectancy.

The NM MOST program allows you to talk about your healthcare preferences with your medical provider and have those wishes documented in a signed medical order.  The NM MOST document is bright green so that it can be seen easily in an emergency.  You should always keep the original NM MOST form in a visible location.  Copies of the document are valid and should be shared with your healthcare providers.

The NM MOST program is not for everyone.  It is designed for individuals who are seriously ill or have a life-limiting illness.  For many people, a traditional advance directive is best.

The NM MOST document is encouraged for seriously ill individuals, but it is not a required form.

Make your plan today.

To do this, have a conversation with your doctor or healthcare provider. Make sure to talk about your health, possible outcomes, and treatment options. Then ask your doctor or healthcare provider to complete the New Mexico Medical Orders for Scope of Treatment (MOST) form based on your choices made during that conversation. You and your doctor or provider need to sign and date it.
